Across the United States, a network of for-profit and private equity-backed companies operates therapeutic and treatment foster care programs that command Medicaid reimbursement rates 3-5 times higher than standard foster care, billing for clinical services that are never delivered to the vulnerable children in their custody. This investigation documents how an estimated $2-4 billion annually in Medicaid funds flows to intermediary companies that designate foster placements as therapeutic without providing actual therapy.

The U.S. Senate Finance Committee June 2024 report, Warehouses of Neglect, confirmed: the four largest RTF operators (Universal Health Services, Acadia Healthcare, Devereux Advanced Behavioral Health, and Sequel/Vivant) systematically billed taxpayers for services while subjecting children to abuse, neglect, and death.
